Notes - Extremely rare player specification shirt believed to have been worn by a Kozák in the friendly match against Belgium played at the King Baudouin Stadium, Brussels on the 17th April 2002. This was the first-ever meeting of the two nations as Slovakia impressed against a team who were heading to the World Cup Finals this summer earning a 1-1 draw thanks to a Janočko free-kick. Bart Goor had put the hosts ahead but the Austria Vienna star scored the equaliser in the 78th minute. Kozák played the first 45 minutes before being replaced. He won 38 caps for the national side following his debut in 1994.
