Notes - Extremely rare player shirt believed to have been issued to or worn by Diego Costa for a Premier League match against Sunderland in the 2014-15 season. The Blues played out a 0-0 draw with the Black Cats in November at the Stadium of Light before running out 3-1 winners at Stamford Bridge on the final day of the season in May where they lifted their fourth Premier League title. Diego Costa joined the side this season from Atlético Madrid and was the signing of the summer as he racked up a superb 20 goals in 26 Premier League appearances en route to the title. Highlights for the Spanish international included a hat-trick against Swansea in September and goals in wins over Burnley, West Ham and Liverpool. He stayed with the side until September 2017 when he returned to Atlético after bagging 59 goals in 120 games for The Blues. Chelsea wore this classy Adidas design both home and away against Sunderland this season with Diego Costa donning short sleeves on each occasion.
