Notes - Very rare player shirt believed to have been issued to Johnson during the 2013-14 period. The midfielder joined The Timbers in 2013 from Real Salt Lake, and made 32 appearances in his debut season, as he helped the side reach the Conference Finals. He remained a key figure in the Portland XI the following year before a severe leg injury in 2014 ended his season prematurely. Johnson won the 2015 MLS Cup before joining Toronto for the start of the 2016 season. The former De Graafschap man has won 45 caps for Canada following his debut in 2005.
