Notes - Rare player shirt believed to have been worn in the Premier League match against Blackburn Rovers played at Ewood Park on the 24th May 2009. This was the last game of the season and The Baggies were already relegated from the Premier League under boss Tony Mowbray. Donk was on loan from AZ and made 19 appearances in all the competitions, including this in this stalemate where he completed the 90 minutes.
