Notes - Home shirt as worn by the side when, in '05, they became the first former winner of the European Cup to be relegated to the third tier of their domestic league
Very rare player shirt believed to have been issued to or worn by a #12 for the Friendly match against DC United, played at RFK Memorial Stadium in Washington DC on 14th July 2004. A close game ended 1-1 before being decided by a penalty shootout which saw the MLS side run out 4-3 winners. Forest would go on to suffer relegation to League One this season after a 23rd place finish in the Championship.
