Notes - Home shirt as worn by Lampard in his first two seasons at the Bridge as the side recorded 4th and 6th place finishes in the Premiership and reached the FA Cup final. The former Hammer joined Chelsea for £11m and missed only one Premiership match for the Blues over two seasons, bagging 11 goals as he grew into the prolific midfielder which would eventually see him become Chelsea's all-time top goalscorer
