Notes - Rare player shirt believed to have been issued to or worn by Cooper for the 2000-01 season. The midfielder joined the side on loan from Nottingham Forrest and made 14 appearances to help the Minstermen to a 17th place finish in the Third Division. Cooper joined the side on a permanent basis at the end of the campaign and stayed with them until 2004 when the side were relegated from the Football League after a 75 year stay,he joined Alfreton Town by which time he had made over 100 appearances for the York.
