Notes - Very rare player shirt believed to have been issued to Cooper during the 2000-01 season. Forest improved to an 11th place finish in Division One this season finishing 6 points off the Play-offs under boss David Platt. Cooper made just two appearances in what was to be his last season at the City Ground coming on as a second half substitute against Sheffield United and Burnley. The midfielder opted to join York City at the end of the season in the search for more game time and later enjoyed spells with Alfreton Town and Eastwood Town.
